Vivaldis Dexthesia Injection is a Schedule H prescription veterinary sedative, analgesic, and preanesthetic agent containing Dexmedetomidine Hydrochloride USP equivalent to Dexmedetomidine 100 mcg/ml, for intramuscular (IM) and intravenous (IV) use in dogs and intramuscular (IM) use in cats. Dexmedetomidine is a highly selective, full agonist at central and peripheral alpha-2 adrenergic receptors, providing dose-dependent sedation, anxiolysis, muscle relaxation, and analgesia. Dexthesia is used as a standalone sedative for minor procedures, as a preanesthetic agent before general anaesthesia induction, and as a component of balanced anaesthesia protocols in small animal veterinary practice.

Dexmedetomidine Hydrochloride USP — equivalent to Dexmedetomidine 100 mcg: Dexmedetomidine is the pharmacologically active dextro-enantiomer of medetomidine (the racemic mixture); as the pure D-enantiomer, Dexmedetomidine has approximately 8-fold higher alpha-2 selectivity than medetomidine and is approximately twice as potent by weight; mechanism of action — Dexmedetomidine is a highly selective alpha-2 adrenergic receptor agonist (alpha-2:alpha-1 selectivity ratio approximately 1620:1); central alpha-2 receptor agonism in the locus coeruleus (the principal noradrenergic nucleus of the brainstem) inhibits norepinephrine (noradrenaline) release via presynaptic Gi-protein-coupled receptor activation — reducing cyclic AMP (cAMP) production, hyperpolarising locus coeruleus neurons via inward rectifier K⁺ channels (GIRK channels), and inhibiting voltage-gated Ca²⁺ channels — producing a natural sleep-like sedation (EEG pattern resembling non-REM sleep, distinguishable from general anaesthesia) and sympatholysis; analgesia mechanism — spinal alpha-2 receptor agonism in the dorsal horn (substantia gelatinosa, Rexed laminae I–III) inhibits substance P release and reduces nociceptive transmission; supraspinal alpha-2 agonism activates descending noradrenergic pain inhibitory pathways; cardiovascular effects — initial brief peripheral alpha-2B receptor-mediated vasoconstriction causes transient hypertension with reflex bradycardia; sustained central sympatholysis then reduces cardiac output, heart rate, and systemic vascular resistance — monitoring of heart rate and blood pressure during Dexmedetomidine administration is essential; respiratory effects — Dexmedetomidine produces minimal respiratory depression at clinical doses compared to opioids, making it advantageous in patients with respiratory compromise; reversibility — Dexmedetomidine sedation is fully reversible with the alpha-2 antagonist atipamezole (Antisedan); preanesthetic use — Dexmedetomidine reduces inhalant and injectable anaesthetic requirements (MAC reduction) by 30–90% in dogs and cats, providing significant anaesthetic-sparing effect and smoother induction and recovery

Safety & Contraindications
- Schedule H Prescription Drug — to be dispensed on prescription of a Registered Veterinarian only
- Monitor heart rate and blood pressure during administration — bradycardia and hypotension may require treatment with atropine or glycopyrrolate
- Use with caution in patients with pre-existing cardiovascular disease, hepatic or renal impairment, respiratory compromise, or debilitation
- Reversible with atipamezole (alpha-2 antagonist) IM at 5x the Dexmedetomidine dose (volume equivalent)
